- If I move, how do I change my address with 1536?
To change your address, you must download and print the Change Of Address form. After filling out your printed form, follow the instructions on the form to FAX or MAIL it.

You need the Honorary Resignation Affirmation form. Members may only process Honorary Resignation if they are in good standing. If a member wishes to be readmitted, member must pay all back dues for the period of his or her withdrawal. The Honorary Resignation Affirmation form is available for download below. Once printed and filled out, it must be faxed to the number on the bottom of the form.
